Diabetes Education

Our Diabetes Education Program is committed to the safety of our clients, staff and the community. While we have entered a new phase of the pandemic with the gradual reopening of health care services, we continue to be committed to keeping everyone safe while balancing the mental and physical wellbeing of our clients for their quality of life.

We believe that ongoing support and education plays an important role in the management of diabetes.  We continue to offer various ways to connect with our clients and support them in their diabetes management:

  • Virtual (OTN) or phone appointments
  • Curbside care appointments
  • Just The Basics Zoom Workshops
  • Outreach at the Cobourg Community Centre, Colborne Hub, and Alderville First Nations
  • Appointments with the Endocrinologist will be held by phone.
  • Our upcoming Annual Northumberland Diabetes Expo on May 10, 2022 

If an in-person appointment is deemed necessary please be aware you will be required to self screen prior to entering and must wear a mask. Home visits can be arranged under certain circumstances. Please discuss with your Diabetes Educator if there is a reason why this might be necessary.

We understand that getting a new diagnosis and dealing with a chronic disease can be overwhelming. Diabetes touches the lives of so many and, unfortunately, more and more people are being diagnosed with this chronic disease every day.

Who We Serve

Adults with Type 1 Diabetes, Type 2 Diabetes, Gestational Diabetes, Prediabetes, those who are at risk of developing diabetes, and those who are on insulin pump therapy.

We are a certified insulin pump therapy clinic offering support to those currently using or looking to start using insulin pumps.

Please note: As a client of our Diabetes Education program, you would continue to be seen by your family doctor in addition to care from our friendly and professional staff.

Our Team

Our Diabetes Team is an interdisciplinary team of Certified Diabetes Educators that includes Nurses, Dietitians, Social Worker, and a Program Assistant.

This team meets with individuals and groups to talk about:

  • What is diabetes and how diabetes medications work;
  • How to self-manage diabetes in order to stay healthier longer and delay complications resulting from diabetes.
  • How to cope with the life stresses that can affect diabetes and how to cope with this chronic disease
