Our newly named Community Health Centres of Northumberland (CHCN) replaces the Port Hope Northumberland Community Health Centre name to better reflect the work that we do across all of Northumberland County, including our many outreach service locations.
We opened in 2009 and continue to improve our programs and services to best meet the needs of our community. Our CHCN delivers primary care (family physicians and nurse practitioners), diabetes education, mental health and addictions counseling, geriatric outreach services, a memory clinic, dental care, and many health promotion and community development activities. We focus on the frail elderly, people struggling with mental health issues and/or addictions, those unable to have even basic housing and food, the lonely and youth at risk.
At our CHCN those who apply should be without a local doctor and facing barriers to accessing health care. We offer services outside of Port Hope in other Northumberland locations including Colborne, Cobourg, and Alderville First Nation. We also conduct outreach work with organizations serving the frail elderly, migrant farmworkers, individuals in high-risk situations such as those with mental health issues, or with clients lacking security in our most basic needs like food and housing.
CHCN makes use of data collected to ensure that we are a valuable contributor to the health and well-being of our County’s most vulnerable people. We are dedicated to providing quick access for urgent issues and to the delivery of care that’s right for each client.
We value the thoughts and feedback of our clients and their families and have a group of clients and family members that meet regularly with the Executive Director and a Board representative.
