CHCN’s clinical programs provide comprehensive, team-based care for some of Northumberland County’s most vulnerable residents. This includes primary care, chronic disease support, diabetes education, seniors’ assessments, caregiver programs, and specialized clinics such as memory assessment and geriatric care.

Caregiver Support Group
Are you looking to connect with other caregivers? This support group meets the first and third Friday of every month and provides information, support, and follow-up for caregivers.

Osteoporosis Support Group
Meet others living with osteoporosis and gain practical information and share common experiences. Monthly meetings cover a variety of topics such as nutrition and safe exercises when living with osteoporosis.
