How to Become a Client
To become a primary care client, you can pick up a client registration form from reception at our CHC or print one off directly from the link below:

The CHC has been open now for over fifteen years, we continue to receive a high number of registrations. Unfortunately, this has caused some delay in getting new patients into the Centre. You will hear from us as soon as we are able to place you with a health care provider. Our goal is to provide you with the best medical care and range of supports to promote illness prevention and good health.
Who We Serve
The Community Health Centres of Northumberland serves all residents within Northumberland County and surrounding area. Special consideration is given to the elderly with chronic conditions, youth at risk, those struggling with mental health issues or addictions and those living on a fixed or limited income without a local physician.
Programs & Workshops
Community outreach programs and workshops, including diabetes education and support, nutrition education, health education and lifestyle programs, are open to everyone and not limited to those registered as clients of the centre.
