CHCN’s clinical programs provide comprehensive, team-based care for some of Northumberland County’s most vulnerable residents. This includes primary care, chronic disease support, diabetes education, seniors’ assessments, caregiver programs, and specialized clinics such as memory assessment and geriatric care.

The Geriatric Assessment and Intervention Network (GAIN) is a Specialized Geriatric Service offered in the Central East Region of Ontario. GAIN supports community-dwelling older adults living with multiple, complex health and social conditions (such as dementia, cognitive impairment, falls, impaired mobility, decreased function, incontinence, multiple medications, etc.).
Through comprehensive assessment and personalized care, our teams assist clients with improving/maintaining their function and independence, so that they can continue living at home.
Our Team
At CHCN, our inter-professional GAIN team comprises of a Nurse Practitioner, a Care of Elderly Physician, a Pharmacist, a Specialized Geriatric Nurse, a Social Worker, a Behavioral Support Nurse, Personal Support Workers, and Nurse Care Coordinators. Additionally, specialist physicians (such as geriatricians) also provide consultation services.
The team works in close collaboration with Primary Care Practitioners (family physicians, nurse practitioners) and Community Support Services to best meet the needs of their clients.

Referrals to GAIN are accepted from primary care providers, hospitals, community organizations and seniors and their family/friend care partners.
