We offer online appointment bookings to rostered CHCN primary care clients currently seeing one of our doctors or nurse practitioners.

Registered Practical Nurses (RPNs)
Book a procedure with one of our Registered Practical Nurses. We currently offer online appointment bookings for the following:
Baby weight check, Blood pressure check, Dupixent, Ear syringe, Immunizations, Methotrexate, Prolia, Suture/staple removal, Testosterone, Vitamin B12, Wart treatment
COVID-19 Vaccines

Mental Health Registered Nurse
To continue with booking an appointment, you must have a primary care provider at CHCN, who has referred you to speak with our Mental Health Registered Nurse, Brooke, and/or have had a previous appointment with Brooke.

Registered Dietitians
Book an appointment with your registered dietitian. For current or newly registered CHCN clients only.
Zoom Meetings Support
Book an appointment online with our Community Health Worker for one-on-one support using Zoom to access CHCN community programs and workshops, such as Active Fit & Fun and Living Well with Diabetes. Upon booking an appointment, our Community Health Worker will contact you by phone.

Why should I consider booking my appointment online?
Online booking allows you to easily and conveniently book an appointment with your healthcare provider, receive optional reminders about upcoming appointments, and helps our healthcare providers and administrative staff to better serve you by reducing call volumes and by collecting helpful information prior to your visit.
Online appointment booking is a simplified and streamlined process, with no registration, login or passwords required. Our online booking portal is mobile-friendly, and you can cancel or re-book using a simple email link.

Do I need an email address to book an appointment online?
While you do not require an email address in order to book an appointment online, it is strongly recommended that you provide one when booking, so that you can easily cancel or reschedule your appointment, and receive reminders about your upcoming appointment.
