POSITION SUMMARY
The Ontario Health Team of Northumberland (OHT-N) is seeking an experienced Registered Nurse to serve as the SCOPE Nurse Navigator on a temporary part-time basis.
Working as part of the Primary Care Access and Attachment Initiative, the SCOPE Nurse Navigator will support the implementation of the SCOPE (Seamless Care Optimizing the Patient Experience) model across Northumberland County. The Nurse Navigator will act as a central point of contact for primary care providers, assisting physicians and nurse practitioners in accessing hospital, community, specialist and diagnostic services for patients with complex health needs.
The successful candidate will work collaboratively with primary care providers, hospital, Ontario Health atHome, community agencies and other health system partners to improve care coordination, reduce avoidable emergency department visits, support timely access to care, and strengthen communication across sectors.
This position will also work closely with the OHT-N Supported Attachment Team to improve access to primary care services for unattached patients and support broader system integration initiatives.
KEY RESPONSIBILITIES
Navigation & Care Coordination
- Serve as a navigation resource for participating primary care providers.
- Respond to requests from physicians, nurse practitioners and clinic staff regarding patient navigation for complex referrals.
- Assist providers in identifying the most appropriate community, hospital and specialist services.
- Coordinate referrals and facilitate access to diagnostics, specialist consultations and community programs.
- Support smooth transitions between primary care, hospital, home care and community services.
- Identify barriers to care and work collaboratively to develop solutions that improve patient flow.
Provider Support
- Develop strong working relationships with participating primary care practices.
- Support providers in navigating available health system resources.
- Assist with referral pathways and follow-up on consultations where appropriate.
- Promote awareness and utilization of the SCOPE model.
- Provide education regarding available programs and services.
Collaboration
- Participate as a member of the interprofessional SCOPE team.
- Build partnerships with hospitals, Ontario Health atHome, community organizations and specialty services.
- Participate in quality improvement initiatives, working groups and implementation meetings.
- Contribute to ongoing evaluation and refinement of SCOPE processes.
Data & Evaluation
- Maintain accurate documentation and activity records.
- Collect and submit program metrics required for reporting and evaluation.
- Monitor trends and identify opportunities for continuous quality improvement.
- Assist with evaluation activities supporting the Primary Care Access and Attachment Initiative.
Quality Improvement
- Support the ongoing implementation and growth of SCOPE within Northumberland County.
- Identify opportunities to improve provider experience and patient access.
- Participate in developing standardized workflows and referral pathways.
- Promote patient-centred, integrated care across the health system.
Salary: $38.46 – $42.85 per hour
Hours: 20 per week
Experience
QUALIFICATIONS
Required
- Current registration in good standing with the College of Nurses of Ontario.
- Bachelor of Science in Nursing (BScN) preferred.
- Minimum five (5) years’ recent experience in primary care, community nursing, acute care or care coordination.
- Demonstrated experience working with patients with complex medical and social needs.
- Experience working within interprofessional healthcare teams.
- Excellent assessment, critical thinking and clinical decision-making skills.
- Exceptional communication, relationship-building and organizational skills.
- Ability to work independently while managing competing priorities.
- Strong computer proficiency including Microsoft Office and electronic medical records.
Assets
- Experience working within an Ontario Health Team.
- Knowledge of the SCOPE model.
- Experience with quality improvement methodologies.
- Knowledge of community resources throughout Northumberland County.
- Experience supporting primary care physicians and nurse practitioners.
Working Conditions
- Temporary Part-Time position with opportunity for extension.
- Hybrid work model, with travel throughout Northumberland County as required.
- Valid Ontario driver’s licence and access to reliable transportation.
