Position: Registered Practical Nurse – Primary Care Clinic
Reports to: Manager, Specialized Programs
Job Summary: The Registered Practical Nurse (RPN) works within an inter-disciplinary team to provide clinical assistance, perform certain laboratory procedures, maintain equipment and medical, laboratory and pharmaceutical supplies, record filing, retrieval and support, and administrative support.

- Performs a broad range of primary care nursing procedures including immunizations, venipuncture, wound care, foot care, ECG testing, specimen collection, and suture/staple removal in accordance with established standards and medical directives.
- Provides effective client support through health screening, patient preparation, health teaching, referrals, appointment coordination, and system navigation.
- Utilizes electronic medical records, telemedicine technology, data collection systems, and office software to support clinical and administrative activities.
Knowledge:
- Demonstrates thorough knowledge of primary care nursing practice, College of Nurses of Ontario standards, medical directives, and evidence-based clinical procedures.
- Maintains knowledge of medical and laboratory terminology, documentation requirements, infection prevention and control practices, and health and safety regulations.
- Understands the principles of health promotion, disease prevention, client education, community resources, and interdisciplinary care delivery.
Judgment:
- Applies sound clinical judgment when conducting assessments, identifying client needs, prioritizing care, and determining appropriate nursing interventions within the RPN scope of practice.
- Uses critical thinking and problem-solving skills to address client concerns, support care planning, and contribute to service improvements and quality outcomes.
- Exercises discretion in interpreting clinical information, identifying risks, and escalating issues to appropriate providers or managers when required.
Accountability:
- Accountable for maintaining accurate, complete, and timely clinical documentation while ensuring confidentiality and compliance with privacy legislation and organizational policies.
- Takes responsibility for safe nursing practice by adhering to occupational health and safety requirements, reporting hazards, and participating in risk management activities.
- Demonstrates professional accountability through ongoing competency development, participation in quality improvement initiatives, accreditation activities, and adherence to regulatory standards.
Service Responsibilities:
Health Service:
- Perform practical nursing procedures that support primary care (e.g. venipuncture, electrocardiogram testing, urine BHCG, foot care, wound care, immunizations and suture/staple removal under the College of Nurses Standards of RPN Practice and Community Health Centres of Northumberland (CHCN) medial directives.)
- As delegated, conduct health screening and assessments, including history and partial physical, for well clients of all ages.
- As delegated, communicate with clients and external providers and services regarding client referrals, test results, follow-up appointments and written prescriptions.
- As delegated, provide health care teaching (e.g. sexual health, breast feeding, smoking cessation).
- Assist with patient flow and prepare clients for encounters with primary health care providers (e.g. taking vitals, assistance with undressing).
- Provide support in system navigation.
- Ensure exam rooms, medication cupboard, medical supplies’ cupboards, labs and clinicians’ offices are kept clean, safe and stocked.
- Maintain the availability of sterile clinical equipment by autoclaving as necessary.
- Maintain and order equipment as necessary.
- Maintain complete, accurate and timely records and encounter data, participate in chart and case reviews and support client education.
- Assist in the development of protocols and procedures in order to provide optimal standardized care to clients.
- Support maintenance of client record systems, correspondence, referral files and computerized registration and encountering.
- Coordinate and support Ontario Telemedicine Network (OTN) appointments, meetings, and educational events
- Work with the clinical team and other health centre teams to identify and take action on community health issues.
Community Health:
- Promote awareness of the Centre’s programs and participation by the community through partnerships with other CHC’s and professionals.
- Maintain a client reference library and actively participate in the preparation and distribution of materials for client education.
Organizational Responsibilities:
- Promote and participate in an interdisciplinary provider model.
- Collaborate with both providers and clients to ensure management, referrals and information.
- Assist with risk management and quality assurance processes.
- Collect statistical information as required by the Centre.
- Participate in the development, planning and evaluation of treatment, education, counseling and health promotion programs.
- Work in a manner that seeks to minimize risk to our clients, the Centre and to Staff. Committed to safety, works in a manner that demonstrates responsibility for following the Centre’s safe work practices and works in compliance with the Occupational Health and Safety Act. Takes responsibility for and is expected to report and unsafe/at risk conditions as well as be accountable for their correction.
- Fully participate in evolving goals of e-health strategies and commit to best practice in information management.
- Incorporate critical thinking practice and a collaborative approach to problem solving and decision making.
- Contribute to the overall mission of the Centre by participating in meetings and on committees to identify, respond, and/or resolve issues or opportunities for improvement.
- Incorporate and contribute to the Centre’s efforts for continuous improvement by identifying, implementing and evaluating policies and practices to support best practice. Actively participate in a valid accreditation process and integrate the principles into work practices.
- Integrate into work functions the principle of respect and value for staff, clients, and the community.
- When appropriate contributes to the Centre’s practices of orientating and training staff.
- Other duties as assigned.
Qualifications:
- Registration as Registered Practical Nurse with the College of Nurses of Ontario in good standing.
- Demonstrated experience working as a Registered Practical Nurse in a community and/or public health setting.
- Demonstrated expertise in medical laboratory procedures.
- Certification and advanced education as required to gain and maintain competency in expanded areas of care (e.g. foot care, smoking cessation, wound care, behaviour support – BSO).
- Thorough knowledge and understanding of medical and laboratory terminology, medical record procedures and charting, and current health and safety hazards and preventative practices.
- Knowledge and understanding of primary prevention, health promotion theory and practice.
- Proficiency in the use of computers and various software applications.
- Proficiency with EMR systems
- Holds current Basic Life Support for Healthcare Providers certificate.
- Demonstrated ability to work effectively within a multi-disciplinary team.
- Valid Ontario drivers’ license and the use of a vehicle.
- Ability to work flexible hours as required
- Completion of a Vulnerable Sector Police Check will be required for employment
