Registered Dietitian: 1.0 FTE 12-month contract with possibility of extension
The Community Health Centres of Northumberland is a non-profit, community-governed organization committed to programs and services that promote wellness and are delivered by an interdisciplinary team.
General Overview: As a member of the Diabetes Education Team, the Registered Dietitian provides clinical care and education to people living with diabetes or at risk for diabetes in individual and group settings; participates in the development, implementation, monitoring, and evaluation of the Centre’s Diabetes Education Program. The Registered Dietitian practices within an interdisciplinary model possessing the ability to work independently and as a collaborative team member within the Diabetes team, the Community Health Centre array of teams, and with other community partners. The Registered Dietitian guides patients on adequate nutrition, healthy eating behaviours, exercise, and blood glucose management.
Responsibilities:
- Provide individual and group counselling to clients living with diabetes, including families/caregivers, in accordance with evidence-based practice guided by Diabetes Canada’s Clinical Practice Guidelines and the College of Dietitians of Ontario.
- Participate in implementing, monitoring, and evaluating diabetes education programs and services including outreach, home visits, and Endocrinology Clinic.
- Provide a thorough nutrition assessment of clients with diabetes taking into consideration the patient’s level of knowledge and readiness, socio-economic, and cultural background.
- Ensure continuity of care by maintaining up-to-date electronic health records and communicating/collaborating with the client’s primary care provider and other service providers and members of the interprofessional team as required.
Experience
- Registration and a member in good standing with the College of Dietitians of Ontario
- Certified Diabetes Educator (CDE) is an asset but not required
- Excellent nutrition assessment, nutrition education, and behavior change counselling skills
- Knowledge of and experience with special nutritional needs/specific diets
- Support an environment of cultural sensitivity, diversity, and inclusion
- Proficiency in the use of computers and various software applications
- Proven strong organizational, interpersonal, and communication skills
- Flexibility of hours – occasional evenings
- Valid Driver’s License to drive in Ontario and access to a vehicle
- Subject to a Police Reference Check as per organization policy
