Job Summary: The Diabetes Education Centre provides diabetes education and management support based on current practice guidelines. As a member of the Diabetes Education Team, the Registered Nurse provides clinical care and education to people living with diabetes.
As a member of the interdisciplinary team, the Registered Nurse provides clinical guidance on health issues related to diabetes and the management of its related health risk factors. Working collaboratively with the Registered Dietitians and with Primary Health Care Providers, the Registered Nurse provides guidance on general diabetes management, which may include guidance on oral and injectable medication (including insulin) doses or regimens, as well as reinforcement of other diabetes management principles outside of lifestyle strategies (e.g. appropriate foot care, recommendations for eye examinations, etc.). The Registered Nurse participates in the development, implementation, monitoring and evaluation of the Diabetes Education Program.

- Communicates in a timely and positive manner with the Program Manager regarding job content, specific objectives and personal performance. Identifies performance goals, objectives and learning needs. Identifies concerns in a timely manner and, in collaboration with the Program Manager, seeks resolution.
- Incorporates and contributes to the Centre’s efforts for continuous quality improvement by identifying, implementing and evaluating policies and practices to support best practice.
- Incorporates and strengthens collaborative and interdisciplinary teamwork.
- Participates as a team member in all staff functions: program planning, team and inter-team meetings, all staff meetings and team building.
- Performs other duties as assigned.
Professional Development:
- Maintains and develops professional competence through on-going professional development and stays current and aware of opportunities to implement new, evidence based methods of client assessment and treatment.
- Participates in self-directed learning to ensure that practice remains relevant by attending professional conferences, e-learning and journal reviews.
- Communicates with relevant professional groups to maintain professional linkages and professional support.
Qualifications:
- Eligible for membership with the College of Nurses of Ontario.
- Certified Diabetes Educator (CDE) or actively working towards CDE designation.
- Excellent health assessment and counselling skills; sound clinical judgment.
- Demonstrated ability to work independently as well as an effective member of an interdisciplinary team.
- Knowledge of the social determinants of health and their affects on well being.
- Knowledge of community resources.
- Experience working with marginalized people as well as the elderly, at risk youth, those suffering from mental health problems and/or addictions.
- Understanding of issues of disadvantaged groups and of rural health needs.
- Experience in program development, implementation, monitoring and evaluation skills.
- Excellent group process and facilitation skills.
- Proficiency in the use of computers and various software applications.
- Excellent interpersonal and organizational skills.
- Ability to work flexible hours, if required.
- Valid Ontario’s driver’s license and the use of a vehicle.

Hours of work: 35 hours per week
Rate of pay: $41-43/hour
Permanent/full time
