The Community Health Centers of Northumberland (CHCN) is a non-profit, community-governed organization that uses a collaborative and interdisciplinary approach to promote the best possible wellness for the residents of Northumberland County.

General Overview:

As a member of the Diabetes Education Centre, the Diabetes Nurse Educator provides clinical care, support and education to people living with diabetes (Type 1, Type 2, Gestational) or at risk for diabetes in individual and group settings; and participates in the development, implementation, monitoring, and evaluation of the Centre’s diabetes education program. The Diabetes Nurse Educator practices within an interdisciplinary model possessing the ability to work independently and as a collaborative team member within the Diabetes Team and the Community Health Centre array of teams and with other community partners.

Experience

Responsibilities:

1.  Provide individual and group support and education to clients living with diabetes – including families / caregivers

2.  Participate in the implementation, monitoring, and evaluation of the diabetes education programs and services, including outreach; home visits and Endocrinology Clinic

3.  Provide a thorough nursing assessment of clients with diabetes taking into consideration the client’s level of knowledge and readiness, socio-economic and cultural background

4.  Ensure continuity of care by maintaining up to date electronic health records and Communicating / collaborating with the client’s primary health care provider and other service providers and members of the inter-professional team, as required.

5.  Participate in the ongoing advancement of best practice for diabetes care, and in the growth and expansion of diabetes education services across Northumberland County.
