Description
The Community Health Centres of Northumberland (CHCN) operates within a comprehensive Model of Health and Wellbeing, which includes the provision of quality Primary Health Care services and programs oriented to meet the specific needs of the people living in the community. We promote Health Equity and Social Justice to reduce health disparities and inequities by advocating for healthier public policy, and we work with community members to ensure Community Vitality and Belonging by building safe and caring communities where everyone is valued.
The Health Promoter creates, coordinates, and implements the health education/promotion/social prescribing programs for Northumberland community residents. This includes facilitating the integration of a health promotion focus in CHCN activities as a member of the Centre’s interdisciplinary team. This position works collaboratively with community groups and partners using a community development model. This position is further expected to be available at hours that are flexible and accommodate our community members and volunteers. This may mean early morning and evening hours as well as weekend time upon occasion.
Responsibilities:
- Performs community population health care needs assessments and works closely with clients and families and community partners to identify emerging population health issues
- Supports the development, implementation, monitoring, and evaluation of illness prevention and health promotion activities and community initiatives in response to the identified needs in the community
- Incorporates the Social Determinants of Health and concepts of the Model of Health and Wellbeing into the practical delivery of health promotion activities
- Establishes and maintains links and develops partnerships both internally and externally to promote the health of the community and the health and wellbeing of individual clients.
- Provides direct education and interventions to both individuals and groups as necessary
- Acquires, develops, and disseminates accessible health education materials and resources
- Acts as a health promotion resource for members of CHCN’s interdisciplinary teams
- Participates in the development of health promotion strategies and campaigns at CHCN
- Advocates for and supports individual and community capacity building through empowerment and community development
- Maintains a communication network with the community and staff by attending community meetings, broader agency network meetings, and other events as needed
- Provides support and supervision to volunteers and students involved in health promotion, as required
- Oversees the recruitment of program facilitators
- Explores service partnerships in collaboration with the management team
- Develops and implements outreach strategies to connect with marginalized community members living with long-term health conditions
- Implements service provision in an appropriate and culturally competent manner to a culturally and socio-economically diverse range of communities
- Supervises, supports, and mentors peers with lived experience
- Develops and implements strategies to engage an active volunteer base and identify potential volunteer leaders
- Facilitates ongoing peer facilitator training and coaching sessions
- Represents community programming with an overall knowledge of CHCN services and strategies, especially when developing new partnerships in conjunction with the management team Coordinates program evaluation activities
- Collects, summarize, and report data aligned with the initiative’s deliverables, including evaluation data
- Provides support to participants and facilitators including form completion, systems navigation, advocacy, social prescribing, and referrals to address needs that are not therapeutic or medical
- Participates as a team member in all team functions program planning, team meetings, facilitator meetings, and partner meetings
- Supports delivery of virtual and in-person workshops as needed
- Oversees and ensures data entry and program database to maintain accuracy and integrity of client and partner information
- Maintains and develops professional competence and learning
- Identifies, with community members, health-related education needs and plans, develops, implements and evaluates health education programming – using various formats such as community events, group programming, use of media, and presentations to large groups
- Participates in infection control measures throughout the facility
- Other duties as required
Experience
- Post-graduate degree from a recognized university in public health, social science, adult education, or a related discipline, or a combination of undergraduate degree and significant experience
- 3-5 years health promotion/education experience in a community-based organization
- Minimum 2 years experience working with marginalized populations
- Excellent group facilitation skills, experience, and knowledge of health promotion and education, community organizing techniques, and program design, implementation, and evaluation
- Knowledge and experience with social prescription programs
- Experience in community outreach, education, and engagement
- Thorough knowledge of community development models and strategies
- Demonstrated commitment to, and knowledge of, community-based health care and resources
- Proficiency in peer education strategies
- Proficiency in the use of computer technology and various software applications
- Strong oral and written communication skills
- Excellent interpersonal and team building skills
- Demonstrated ability to work effectively in a multi-disciplinary team environment
- Experience in working with vulnerable and other priority populations
- Knowledge/experience working in chronic disease self-management programs
- Police clearance documentation will be required for employment
- Proof of triple COVID-19 vaccination status will be required for employment
- Valid Ontario driver’s license and reliable vehicle is required
