The Community Health Centres of Northumberland is a non-profit, community-governed organization committed to programs and services that promote wellness that are delivered within an interdisciplinary team.
The Community Health Centres of Northumberland Diabetes Education Program (CHCN DEP) provides free specialized diabetes education (included cooking classes) to all clients in individual, team and group forums. Foot Care Services are also available to clients who are unable, due to financial constraints, to access these services elsewhere. The Program currently has an opening for: Diabetes Foot Care Nurse (3 days/week).
Responsibilities
- Conducts all aspects of foot care services in accordance with the highest quality care
- Standards of Practice established by the CNO
- Ensures all supplies and related equipment are properly maintained, cleaned and available
- Ensures appropriate infection control standards are followed in relation to the CNO, MOHLTC and CHCN
- Maintains accurate and timely client records and documentation in relation to CNO Practice Standards and CHCN policies
- Collaborates with Diabetes Education Team, and works together with client, team and primary care provider to ensure comprehensive care and communication regarding foot care
- Provides health teaching for best possible diabetes foot care management/self-management
- Reviews and evaluates clients’ CHCN Financial Means Assessments in order to determine appropriate clients and facilitates access to other foot care services for those not qualified
- Collects and reports foot care data for LHIN reporting, in accordance with the predetermined schedule
- Conduct occasional foot care outreach activities, as developed by the Diabetes Education Program
- Accountable for being knowledgeable of all policies and procedures related to their employment and work environment
- Promotes the Diabetes Foot Care Program as part of the overall Diabetes Education Program mandate
Experience
- Registration with the College of Nurses of Ontario (CNO) RN/RPN
- Minimum 2 years’ recent experience as a practicing foot care nurse (preference to those with diabetes foot care experience)
- Advanced Foot Care Certificate (proof will be required)
- Experience with clinical electronic medical record/scheduler an asset
- Ability to work independently and collaboratively in an Interprofessional environment
- Current driver’s license and access to a reliable vehicle and a willingness to travel
- Police clearance documentation will be required for employment
- Proof of triple COVID-19 vaccination status will be required for employment
