CHCN’s clinical programs provide comprehensive, team-based care for some of Northumberland County’s most vulnerable residents. This includes primary care, chronic disease support, diabetes education, seniors’ assessments, caregiver programs, and specialized clinics such as memory assessment and geriatric care.

What is the MINT Memory Clinic?

MINT Memory Clinics are Multispecialty INterprofessional Teams of family doctors, nurses, social workers, pharmacists, and others who are specially trained in caring for people living with dementia and other memory difficulties. The team provides assessment, early diagnosis and ongoing care in a primary care setting.

The MINT Memory Clinic provides a team approach for assessment, early diagnosis, treatment, and ongoing support of problems associated with memory loss. Our team includes a variety of professionals (doctor, occupational therapist, client support worker, pharmacist, social worker, and nurse) who will conduct a thorough assessment, and communicate their findings to your primary care provider (family doctors or nurse practitioners).

Our team is comprised of members from The Community Health Centre of Northumberland (CHCN), the Northumberland Family Health Team (NFHT), and the Alzheimer Society of Peterborough, Kawartha Lakes, Northumberland and Haliburton (ASPKLNH). We also work closely with other groups like the Alzheimer Society, and specialist physicians to ensure your care needs are met.

For many memory conditions, early diagnosis and treatment can help to maintain and support cognitive health and quality of care.

What can you expect?

  • In the Memory Clinic, you and your family members will meet with the team of health care staff who will work with you to complete a full assessment. The information collected will provide you and your family doctor or nurse practitioner with a better understanding of your cognitive health, and recommend steps you and your family can take to support you.
  • Your appointment will take approximately 2.5 hours.
  • You will be asked to bring along a family member or someone who knows you well.
  • Driving safety will be assessed.

How to get an appointment

If you would like an assessment by the MINT Memory Clinic team, please ask your family doctor or nurse practitioner for a referral: MINT referral form

Once the referral has been received, you will be contacted by the team to begin the process of organizing an appointment date.
