During the COVID-19 pandemic, we are continuing to accept new referrals and provide comprehensive diabetes services to clients as per below:
Curbside Diabetes Care: in Port Hope
Currently available on Mondays – by appointment only
- Supply pick up
- Glucometer training
- Meter and insulin pump downloads
- Foot screenings
Outreach: – by appointment only
Colborne – Wednesdays – 34 Victoria Street
Alderville First Nations –In person with extra precautions or video calls
Cobourg – location currently closed
Home Visits
