The Community Health Centres of Northumberland (CHCN) is a community-centred primary health care organization. With a vision rooted in providing comprehensive primary care services, delivered by an interdisciplinary team of professionals to promote healthy communities, we offer a range of community-based services including but not limited to diabetes education, mental health counselling, dental care and geriatric services. Our work is grounded in the Model of Health and Wellbeing, inclusive of community vitality and belonging, health equity, and social justice.
The CHCN has a current opening for a part-time, contract Trans Health Peer Supporter.
Responsibilities:
- Co-facilitating support group for gender diverse clients
- Providing one on one peer support and advocacy
- Liaise, promote and develop partnerships with service providers
- Participate in public awareness and education
- Provide support to the Gender Affirming Care program, as required, and according to established CHCN guidelines
- Work in collaboration with CHCN staff and external agencies as is necessary to ensure a collaborative approach to meeting health and wellbeing needs
- Recognize potential and real barriers for client access to care and identifying opportunity for system improvement
- Collect and accurately document client information in the electronic medical record in accordance with CHCN policies, procedures and privacy guidelines
- Respond to and triage client and non-client phone calls for information, health education, and/or resources as appropriate, and determine the appropriate actions including forwarding calls or messages to providers
- Communicate effectively with all team members within the Centre
- Maintain knowledge of the CHCN’s emergency procedures
Other Duties and Responsibilities:
- Assist in orientation and training of new staff members and students, as necessary. Participate in hiring panels when requested to do so.
- Participate in staff education, as required
- Attend staff meetings, as required
- Participate in program planning, evaluation and quality improvement activities as well as completing program statistics
- Participate in formal self-evaluations by identifying strengths and limitations
- Seek appropriate supervision/consultation of own actions
- Adhere to all CHCN policies, protocols, and procedures, and promotes best practice
- Perform other duties and functions related to the job description, not exceeding the above stated skills and capabilities
Experience
Qualifications:
- Thorough understanding of transgender health issues
- Lived experience as a Trans, Non-Binary, Two-Spirit, Intersex and/or Gender Diverse individual
- Previous experience providing peer support to Trans, Non-Binary, Two-Spirit, Intersex and/or Gender Diverse persons
- Previous experience as a helper/mentor
- Previous experience and training as a counsellor preferred
- Previous experience in community and program development an asset
- Proven ability to promote team work
- Previous experience with support groups preferred
- Previous experience in public speaking an asset
- Completion of high school education or equivalent; an equivalent combination of education and lived experience may be considered
- Minimum two years’ experience in a health, community or social services setting is an asset
- Excellent computer skills, including working knowledge of Microsoft Office
- Ability to operate information technology equipment, as required
- Excellent communication skills, both verbal and written
- Excellent telephone skills (answering, transferring, and professional phone etiquette)
- Demonstrated ability to organize and prioritize workload
- Ability to respond to a variety of simultaneous requests
- Demonstrated ability to work independently
- Demonstrated ability to follow directions and work as a member of a team
- Use of a positive approach in all aspect of job responsibilities
Salary: $23.00 per hour
Contract/Hours: Up to 21 hours per week, until December 31, 2024
Hours of Work: Days, Evenings
