The Community Health Centres of Northumberland (CHCN) operates within a comprehensive Model of Health and Wellbeing, which includes the provision of quality Primary Health Care services and programs oriented to meet the specific needs of the people living in the community. We promote Health Equity and Social Justice to reduce health disparities and inequities by advocating for healthier public policy, and we work with community members to ensure Community Vitality and Belonging by building safe and caring communities where everyone is valued.
The Community Health Worker provides support, outreach, and education, and administers and implements a variety of community based programs and services, to assist clients to maximize their overall health and wellbeing. This position works collaboratively with community groups and partners using a community development model. This position is further expected to be available at hours that are flexible and accommodate our community members and volunteers. This may mean early morning and evening hours as well as weekend time upon occasion.
Responsibilities
- Co-ordinate, facilitate, implement and evaluate current social, educational and outreach programs at the CHCN working with community members, volunteers and with those most at risk to develop an inclusive and welcoming environment.
- Identify programs and resources needed to support community members and outreach activities by developing and maintaining informal and formal community networks. Including using various formats such as community events, group programming, use of media, and presentations to large groups.
- Supports and facilitates the delivery of virtual and in-person workshops as needed
- Work from a Social Determinants of Health perspective to improve health outcomes for individual clients and/or for the development of community health and well-being, and to reduce health inequities.
- Maintains a communication network with the community and staff by attending community meetings, broader agency network meetings, and other events as needed
- Provides support, training, mentorship and supervision to volunteers and seasonal staff involved in community programs, as required
- Implements service provision in an appropriate and culturally competent manner to a culturally and socio-economically diverse range of communities
- Collect, summarize, and report data aligned with the initiative’s deliverables, including evaluation data
- Oversees and ensures data entry and program database to maintain accuracy and integrity of client and partner information
- Participates as a team member in all team functions program planning, team meetings, facilitator meetings, and partner meetings
- Maintains and develops professional competence and learning
- Other duties as required
Experience
- Undergraduate degree in a relevant discipline from a recognized university or an appropriate combination of education and relevant experience
- 3-5 years’ experience in a community-based organization
- Minimum 2 years’ experience working with marginalized populations
- Excellent group facilitation skills, experience, and knowledge of program design, implementation, and evaluation
- Experience in community outreach, education, and engagement
- Demonstrated commitment to, and knowledge of, community-based health care and resources
- Proficiency in the use of computer technology and various software applications
- Strong oral and written communication skills
- Excellent interpersonal and team building skills
- Excellent organizational skills and ability to multi-task
- Demonstrated ability to work effectively in a multi-disciplinary team environment
- Experience in working with vulnerable and other priority populations
- Police clearance documentation will be required for employment
- Proof of double COVID-19 vaccination status will be required for employment
- Valid Ontario driver’s license and reliable vehicle is required
