Board of Directors Recruitment

We are seeking applications from Northumberland County Residents to join our 2022/23 Board of Directors

 

Our Community Health Centre is a not-for-profit corporation that adheres to all five of the following domains:

Team-Based Primary Care:   We offer high-quality primary care through a collaborative team approach. Family physicians, nurse practitioners, nurses, dietitians, social workers, health promotion, an occupational therapist, dentists, dental hygienists, and other professionals provide services in a team environment

Integrated care & support: We integrate team-based primary care with health promotion programming, illness prevention programs, and community development initiatives

Community-Centered: We focus on the most appropriate services and programs for our local community, whether that means our geographical service area (Northumberland County) or community of individuals with common characteristics (such as complex healthcare needs)

Address Social Determinants of Health: We support individuals, families, and communities to achieve health by actively addressing “social determinants of health” such as poverty, access to shelter/housing, education, language barriers, and other factors that have a direct impact on health, including access to appropriate healthcare services

Health Equity and Social Justice: In our service delivery model of health and wellbeing we address the challenges and barriers that prevent people from having equitable access to services and quality care
