Position Reports To: Clinical Director

Location: 99 Toronto Road, Suite 101, Port Hope, Ontario L1A 3S4

Reporting to the Clinical Director, the Nurse Practitioner (NP) provides primary health care with a strong emphasis on health promotion and disease prevention within the scope of practice and standards of care outlined in the Expanded Nursing Services for Patients Act and Regulations and Practice Expectations Specific to Registered Nurses in the Extended Class (NPs). The NP diagnoses and manages common health problems and diseases both independently and in collaboration with the client and other providers within the circle of care. The NP works as part of an interdisciplinary team to set priorities, develop, implement, monitor, evaluate programs and address issues in the community that affect health. The NP is responsible and accountable for their practice.

Hours: 35 hours per week
Hours of Work: Days, rotating on-call schedule
Start Date: April 1, 2025
