Description
The Director, OHT-N will report to the Co-Chairs of the Collaboration Council, and will be responsible for operationalizing and implementing the OHT-N strategy, by coordinating and supporting all aspects of the development, implementation and growth of the OHT. In this role, the Director will also provide operational and project management support to all OHT initiatives. The candidate is a self-starter who works well independently and collaboratively; demonstrating the excellent communication skills required to work effectively with a diverse group of OHT-N members & partners as well as the Ministry of Health and other key funding stakeholders.
Specific Duties include but are not limited to:
- Provide support to the Collaboration Council, both strategically and administratively. Includes development of effective Collaboration Council briefings with clear and succinct definition of the issues
- Oversee all activities and reporting requirements related to MOH Transfer Payment Agreement including developing budgets, preparing year-end reporting, tracking progress towards milestones & performance indicators.
- Work with patient, caregiver & provider representatives to redesign an integrated model of care
- Support the communication of the strategic directions of the OHT-N and how they align with Quadruple Aim
- Seeks opportunities for additional funding, and initiate applications for funding on behalf of the Collaboration Council
Experience
- A minimum of 5 year’s progressive leadership experience in a health care or related environment working with dynamic and diverse teams and managing complex, large scale projects
- Bachelor’s/Master’s Degree in a health related field preferred
- Proven project management experience (PMP an asset)
- Experience with measurement, evaluation and improvement of program outcomes. Strong understanding of population health, health equity and integration preferred.
- Excellent analytical and problem solving skills.
- Experience developing and managing budgeting and reporting of financial and other key metrics
- Strong written, verbal communication and interpersonal skills
- Proven ability to develop and foster strong stakeholder relationships including building strategic relationships across organizations and working collaboratively with patients and caregivers to redesign care.
- Excellent organizational skills and ability to adapt to continually changing priorities and deadlines.
