The Community Health Centers of Northumberland (CHCN) is a non-profit, community-governed organization that uses a collaborative and interdisciplinary approach to promote the best possible wellness for the residents of Northumberland County.
General Overview:
The Community Connector, under the supervision of the Community Health Promoter, will work with our most marginalized and vulnerable clients in Northumberland County to help connect, support, and facilitate a client’s journey. The Community Connector will provide direction, information and resources to help a client achieve improved quality of life. In partnership with the Social Prescribing Committee, the Community Connector will help connect clients to various psychosocial supports at the Centre and in the community.
Responsibilities:
- Provide person-centred support to clients who require access to psychosocial supports at the Centre and in the community. The aim of this is to:
- Build confidence
- Reduce isolation
- Provide advocacy
- Support/enable self-care
- Engage, listen and build relationships with clients of all ages, genders and backgrounds
- Conduct initial assessment of needs with client, set goals and develop solutions to issues they care about to improve health
- Provide on-going support and communication with the client
- Receive referrals from Primary Care and Allied Health Professionals and communicate with staff on progress
- Conduct internal and external referrals to health and social services when appropriate
- Develop and maintain an in-depth understanding of what services are available in the community for our clients and families
- Facilitate, develop, and implement a virtual map of social and wellness services in the community
- Develop appropriate metrics, collect feedback from clients and staff, and analyze the value and sustainability of the program
- Establish and develop links with community groups, services and agencies in Northumberland County
- Work alongside community organizations to resolve issues of barriers to access
- Liaise with community organizations to proactively share information about service availability and changes within the sector
- Maintain accurate records of all work undertaken, track project budgets, schedules and ensure deadlines are met
- Maintain records in accordance with data protection guidelines, and maintain client confidentiality at all times
- Attend primary care and other Allied Health Team meetings to discuss Social Prescribing and how it can be implemented in their practice
- Recruit and train Health Champion Volunteers to the program to support sustainability
- Report to the Community Health Promoter on a regular basis
- Any other reasonable duties as required
Experience
- Education in social services or related field, or 2-3 years of equivalent experience
- Experience of supporting individuals working in community work, health, social care or information and advice
- Experience of working with vulnerable populations
- Excellent interpersonal and advocacy skills and an ability to quickly relate to people in a non-judgemental way
- Strong organizational and multi-tasking skills.
- Excellent listening and communication skills
- Empathy, emotional resilience, non-judgemental
- Ability to work creatively, independently and autonomously in a complex community environment
- Ability to manage a heavy caseload and meet deadlines
- Ability to easily attend off-site meetings, travel to satellite locations, conduct home visits, and participate in community engagement activities (valid Ontario driver’s license and the use of a vehicle highly recommended).
- Subject to a police reference check as per organization policy.
Salary: $23.00 per hour
Contract/Hours: 23 hours per week (Flexible work week/Part-Time/Contract)
Anticipated start/end date: August 14th, 2023 to June 30th, 2024
