Geriatric Assessment & Intervention Network (GAIN)

The Geriatric Assessment and Intervention Network (GAIN) is a Specialized Geriatric Service offered in the Central East Region of Ontario. GAIN supports community-dwelling older adults living with multiple, complex health and social conditions (such as dementia, cognitive impairment, falls, impaired mobility, decreased function, incontinence, multiple medications, etc.).

Through comprehensive assessment and personalized care, our teams assist clients with improving/maintaining their function and independence, so that they can continue living at home.

Our Team

At CHC-N, our inter-professional GAIN team comprises of a Nurse Practitioner, a Care of Elderly Physician, a Pharmacist, an Occupational Therapist, a Social Worker, a Behavioral Support Nurse, Personal Support Workers, and Nurse Care Coordinators. Additionally, specialist physicians (such as geriatricians) also provide consultation services.

The team works in close collaboration with Primary Care Practitioners (family physicians, nurse practitioners) and Community Support Services to best meet the needs of their clients.
